0106(10,a,b): Licensor was unable to verify food workers card on site for a lead staff member. 0106(3,a,b,c): Licensor observed a staff that was in ratio without a completed school age basics. 0106(9,a,b): Licensor could not verify on site a current CPR and first aid for a staff.
